Effective April 29, 2020 the Maryland Children’s Whole Life application packets were updated, please make sure you begin using the new application packet by checking the form number. App pack form numbers: Old: ICC17L663A New: ICC18L672A The new paper application packets are available and can be found on Sales Professional Access or ordered through your normal ordering channels.
