Effective April 29, 2020 the Maryland Children’s Whole Life application packets were updated, please make sure you begin using the new application packet by checking the form number.
App pack form numbers:
- Old: ICC17L663A
- New: ICC18L672A
The new paper application packets are available and can be found on Sales Professional Access or ordered through your normal ordering channels. The electronic application is updated with the correct forms.
Transition Rules:
Children’s Whole Life applications submitted after June 1, 2020, must use the new Maryland application packet. Maryland application packets with the old form number will not be accepted after June 1, 2020.
