Effective January 1, 2017, a secondary addressee form (the Secondary Addressee form – 3333) will be added to Rhode Island application kits and will be available from the Supplies section of the MyNGLIC.com agent website.

The addition of this form will help you stay compliant with the state requirement to offer clients the right to a secondary addressee— to designate an additional person to receive notification of possible lapses or cancellation in coverage.
