Reminder – Maryland Form Required January 1st


Effective January 1, 2017, Maryland agents are required to submit the Model Replacement form when a client indicates on their application that they have existing insurance coverage, even when they do not intend to replace their existing coverage.

Beginning January 1, 2017, policy issuance will be delayed if a client indicates they have existing coverage and the form is not included with the submitted paperwork. This form requirement is going into effect to be compliant with the State of Maryland’s Model Replacement rule.

The required Model Replacement form is:

  • Currently included in Maryland application kits, and
  • Available to download and print from the Supplies link from your agent page.
