Each individual applicant allows agent’s name to be entered in drawing. Annualized premium per applicant must be no less than $800. Qualifying
products include MutualCare® Custom Solution or Secure Solution, Thrivent LTCi, National Guardian Life (NGL) EssentialLTC, Guarantee Trust Life
(GTL) Critical Cash, Critical Care, Recover Cash, Short-Term Home Health Care. Field declines, clearly uninsurable cases, and Workplace business will
not count toward drawing. You must be a GoldenCare-appointed agent to participate and collect prize(s), with business (if ultimately issued) generating
some override to GoldenCare. Incentive prizes paid in addition to GoldenCare Reward bonus(es) for select business.
