As you may know, many of our online forms have been set up to be completed electronically in an effort to make them easier to fill out. For the greatest success in electronically completing forms, please follow the steps listed below:

1. Download the forms directly from the Forms Portal. (It is always wise to double check the Forms Portal to ensure you have the most current forms each time you use them.)
2. Save the form to your desktop before completing any of the fields.
3. Re-open the saved form from your desktop and complete the required interactive fields.
4. Save and print the form for signatures.
5. Submit to the home office for processing.

Keep in mind that you must download and save the forms before completing any of the fields. If the form is opened in your browser window when you fill in the information and then save and print, the information will not be captured and you will have to start over.
