Reminder:
All Mutual of Omaha Rx paper enrollment applications must be received within 48 hours of the application sign date to our processing center in Pennsylvania.

The enrollment application and Scope of Appointment (SOA) are submitted to different locations; it is critical that you submit the forms via the appropriate methods noted on the Agent Checklist in your enrollment packet.

Paper Enrollment Application:
Enrollment applications only must be submitted to either the Fax number or mail address below. It is highly recommended that the enrollment application form be submitted via Fax.

Scope of Appointment
A copy of the SOA only must be submitted to the Mutual of Omaha home office.

Do not use the email or Fax number provided for submission of the SOA to submit a paper enrollment application. This may have a negative impact on the processing of your client’s application.
