REMINDER: DVH Policy Change Rule

Policy changes to or from a Basic/Premiere PPO Dental plan to Prime (DVH) may only be made effective upon the existing Policy Anniversary Date. Applications may be submitted up to 90 calendar days prior to the Policy Anniversary Date. The change to the newer plan will not be effective until the Policy Anniversary Date of the replaced plan.

Additionally, commissions paid will be durational, and there will not be an advance.
