REMINDER: We have revised your states Short-Term Home Health Care application, to include questions for the Critical Accident Rider. This change is effective immediately. The Agent and E-Application platforms have also been updated.

You can access the new Short-Term Home Health Care applications online through GTLink, from our website (www.gtlic.com) by selecting Agents and then GTL Agent Login, under your own name/password. Once logged in, select GTLink (PDF Forms).
