The Massachusetts electronic delivery and signature consent form to be used with GTL health products has been revised. Effective July 1, 2018, you are required to use the revised form that is attached to the back of product applications, and give it to the client at the time the application is taken. The products that require this form are:

Advantage Plus Hospital Indemnity — APPH10-14-MA (Stock # 15A462)
iGAP Guaranteed Issue Accident Coverage — ENR10-15(MA) (Stock # 15A835)

You can access the new applications with the form attached online through GTLINK, from GTL’s website (www.gtlic.com). E-applications will automatically be updated to reflect the new form.
