Policy benefit features and limits are shown below:*

• Issue Ages of 18-89
• Facility Care and Home Healthcare Benefits, funded separately so using one won’t impact the other
• Simple Plan Design: Three Basic Options:
  • Choice 100: $100 Daily Maximum
  • Choice 200: $200 Daily Maximum
  • Choice 300: $300 Daily Maximum
• Benefit Period Options: 180 or 360 Days (Lifetime Maximum = Covered Days X 2)
• Elimination Period Options: 0 or 20 days
• Optional Riders Available:
  • Inflation Protection Rider (5% Simple)
  • Home Care Booster Rider: Doubles the amount of the Home Healthcare benefit
  • NOTE: If a rider is selected it must follow the base plan selection. I.e. Choice 100 base policy must be paired with a Choice 100 rider
• Care Transition benefit: Up to 10 days of immediate Home Healthcare assistance provided by a family member or friend until professional assistance is available
• Restoration of Benefits: Both the Facility Care and Home Healthcare benefits reset once each per policy lifetime after recovery and care is not needed for at least 180 days
• Indemnity Benefits paid to insured and can be used however they choose
• 15% Spouse Discount if both apply and are issued identical policies
• No network requirements; client can choose Home Care providers and facilities
