Washington D.C. applications for Mutual of Omaha’s Critical Illness, Critical Illness at Work, Lump Sum Cancer and Guaranteed ADvantage Accidental Death* have been revised to comply with the Affordable Care Act.  These applications also must now be accompanied by a DC Minimum Essential Coverage Attestation form.
